What does Foucault mean by totality?

I'm reading Madness: The Invention of an Idea (also known as Mental Illness and Psychology) by Michel Foucault for my Intro to Philosophy class, and I honestly have no clue what he's talking about. I am in no way familiar with psychological jargon, and it seems like he is referring to something very specific here as opposed to just the normal useage. Thoughts?
